Advanced musculoskeletal physiotherapy: Barriers and enablers to multi‐site implementation

ABSTRACT
Advanced musculoskeletal physiotherapy (AMP) services are a safe, effective model of care, but without broad-scale healthcare implementation to date. The aim the present study was identify barriers and enablers 12 AMP from perspective clinical staff.In qualitative study, participants (physiotherapists), different networks (seven metropolitan, three regional, two rural), were included. Their departments implemented (orthopaedic postoperative joint replacement review, n = 10; general orthopaedic, 1; emergency, neurosurgery 1) over 12-month period. Participants completed structured survey specifically designed for study. Thematic analysis used, with themes mapped validated Theoretical Domains Framework.Nine major emerged data regarding staff. These were: demand/capacity; care; organization; stakeholders; communication; planning processes; evaluation; workforce; learning assessment framework. Important included engagement buy-in key stakeholders medical staff, well-established frameworks training operational frameworks. Barriers competitive funding environment, issues that hindered communication. knowledge, skills, availability, motivation experience advanced physiotherapists had large impact on implementation.The identified number factors should be considered successful across or wider networks.
