ABSTRACT
The Tarlov perineural cyst, is often an incidental finding in a pelvic ultrasound examination and may mimic common gynecological mass. We report case of complex adnexal mass incidentally diagnosed women 41 years old. patient was refered to our center with showing oval set 6 cm two including cysts 4 e 3 respectively, the Douglas pouch probably arising from prolapsed left ovarian. Tumor markers were negative. scan showed unchanged formation after mestrual cycle. At transvaginal we suspected that not ovary. MRI without contrast medium, performed approximately 1 week, described periradicular sacral multiple gross cystic formations, which largest one presacral, paramedian predominantly extraforaminal ; therefore made diagnosis “Tarlov cysts”. etiology still unknown could include inflammation, trauma, congenital origin, degenerative processes, genetic predisposition; incidence 1%. are located mainly lumbosacral level. They frequently asymptomatic but present abdominal pain, back intestinal genitourinary disorders. appear as multilocular localized adjacent sacrum mobility during breathing (because extra peritoneal localization). differential include: benign malignant ovarian lesions, paraovarian cysts, hydrosalpinx, abscess, hematoma. Transvaginal allow correct management reduce morbidity discomfort.
