ABSTRACT
Physeal distraction facilitates metaphyseal bone tumor resection in children and preserves the adjacent joint. The technique was first described by Cañadell. Tumor procedures allowing limb-sparing reconstruction have been used increasingly recent years without compromising oncologic principles.We report our results with Cañadell's assessing control, functional outcome, complications.Six consecutive primary malignant tumors underwent physeal as a part of resection. location distal femur four patients, proximal humerus one patient, tibia patient. outcome evaluated after minimum 18 months (median, 62 months; range, 18-136 months) using Musculoskeletal Society (MSTS) score Toronto Extremity Salvage Score (TESS).At latest followup, five patients were alive disease-free had died from metastatic disease. All resections resulted local control; there no recurrencies. mean MSTS 79% (range, 53%-97%) corresponding TESS 83% 71%-92%). In case, postoperative infection required amputation lower leg. distractions successful except for patient whom rupturing into tumor. This situation salvaged transepiphyseal resection.We consider useful tool armamentarium to treat that are close proximity an open physis.Level IV, therapeutic study.
