Revisiting Posterior Fixation Sutures Surgery: Unveiling Novel Approaches for Primary Management of Diverse Esotropia Cases

ABSTRACT
Strabismus, specifically esotropia, presents a significant challenge in ophthalmic surgery, while several treatment options exist. This study aims to evaluate the results of posterior fixation sutures (PFS) on medial rectus as primary approach for some types esotropia. The medical records consecutive patients who underwent surgery esotropia over 11 years and had at least 1 year follow-up were reviewed retrospectively. Patients classified into one three deviation: infantile (IE), partially accommodative (PAE) basic (BE) esotropias. An alignment within 16 prism diopters (PD) orthotropia was successful outcome. A total 404 included: 67 IE, 180 PAE 157 BE. Before deviation greater than 30 PD present 88.1% 80.1%, 50 66.5% 52.9% (near distance, respectively). In BE group, PFS baseline smaller number cases (75%) compared other two groups (versus 86.6% [IE] 88.3% [PAE], p = 0.002). need an additional procedure significantly higher group (44.8% vs. 18.9% 24.8%, < 0.001). Final surgical success achieved 95.3% all patients. Orthotropia 19.4% 29.6% 25.5% cases. without recession proved first-line subtypes evaluated this study.
