The Gap Is Getting Closer: Trends in Absolute and Relative Black-White HIV Mortality Disparities in the USA (1999–2023)

ABSTRACT
Abstract Background This study analyzes trends in HIV-related mortality disparities between Black and White populations the USA from 1999 to 2023. Methods CDC WONDER data were utilized assess absolute disparities, measured as age-adjusted rate difference, relative ratio, Black-White HIV mortality. Trends analyzed using Joinpoint regression, with average annual percentage change (AAPC) serving a summary measure for entire period (APC) used evaluate magnitude of within specific intervals. Results The disparity showed consistent decline, an AAPC − 6.46% ( p < 0.001). Similarly, significantly decreased, 1.10% A detailed analysis revealed notable inflection point 2007, marked by initial increase 2007 (APC 0.96%, = 0.002), followed decline 2023 2.12%, findings both females males consistently demonstrated reductions aligning overall trend. Among age categories, 45–54 group greatest improvement (− 8.11%) 2.65%) while 25–34 least, slight (+ 0.23%, 0.66). Conclusion shows significant progress reducing reduction inequalities is more modest, emphasizing need continued strengthened public health efforts.
