ABSTRACT
Life expectancy is rising in developed countries. The impact of age on melanoma characteristics unclear, but it seems that melanomas the elderly have distinct features affecting management and outcomes. To compare clinical histopathological younger patients. A retrospective population-based study analyzed observed between 2007 2022 was made southern Seville health area (Spain). Patients were divided into two groups: <65 ≥65. Data collected from histories. Among 431 primary cutaneous melanomas, 33% patients ≥65-years. Elderly had more head neck (37.8% vs. 14.9%; p < 0.001), larger lesions (1.3 0.9 cm; ulcerated (17.8% 8.8%; 0.012), higher Breslow thickness (1.03 0.65 mm; 0.01) than No differences found number mitoses or invasions. Stage 0 advanced stages (II/III/IV) frequently ≥65-years (29.3% 23% 27.1% 15.7%, 0.001 respectively). Fewer wide excisions (28.4% 5.6%, sentinel lymph node biopsy (17.6% 2.4%, adjuvant therapy (11.9% 2.1%, 0.001) performed retrospective, primarily covering last 10-years, with older data missing. Key risk factors like nevi family history not collected. Melanomas diagnosed at initial despite having worse prognostic compared occurring people.
