ABSTRACT
Several serological tests for specific precipitin or IgG are available to demonstrate type III hypersensitivity reactions Aspergillus species and essential infectious fungal disease diagnosis. These assays also important allergic bronchopulmonary aspergillosis (ABPA) diagnosis; however, their concordance in ABPA was not well studied.Fifty-two patients diagnosed based on ISHAM criteria were enrolled. Precipitins fumigatus, niger, flavus, terreus measured using Ouchterlony double immunodiffusion ImmunoCAP method, respectively. A. fumigatus-specific determined complement-fixation (CF) method.Forty-eight percent of cases double-positive (ImmunoCAP), whereas 3 (6%) 14 (28%) positive alone, Kappa coefficient between these measurements 0.32, suggesting poor concordance. Double-positive more likely present: sp. sputum culture, lower pulmonary functions, peripheral blood eosinophilia, higher total IgE titer than precipitin-negative cases. only 8 (15%) The presence associated with antibodies other spp., cross-reactivity.Positive rate (ImmunoCAP) superior (CF), but relatively noted (ImmunoCAP). Positive fumigatus suggests active diseases. Cross-reactivity may exist different spp. Therefore, the results diagnosis should be carefully evaluated.
