Lung ultrasound is a useful tool for diagnosis and follow-up of diseases in critically ill neonates. Its use increasingly widespread thanks to its advantages over other imaging tests the rapidly growing body evidence support it, "point-of-care ultrasound" (POCUS) has become key component neonatal guidelines. The objective this special article present foundations established diagnostic therapeutic applications lung ultrasonography as well introducing new applications. The Ultrasound Section Neonatal Ultrasonography Working Group Spanish Neonatology Society summarised current scientific evidence. describes sonographic features most common respiratory diseases, discusses some care (such prediction admission need surfactant, ultrasound-guided procedures or monitoring development premature infants) proposes introduction scenarios which not quite at present, such resuscitation management. This reaffirms usefulness guiding diagnosis, clinical decision-making prognosis facilitating procedures.Lung should be gold standard Therefore, training included educational curriculum neonatologists protocols. Research on subject continue pursued with performance rigorous multicentre studies increase quality
