ABSTRACT
Long-term follow-up (LTFU) care, although endorsed, is not available for the majority of adult survivors childhood, adolescence and young (CAYA) cancer. Barriers to implementation include lack time, knowledge, personnel funding. Sustainable solutions are urgently needed address needs CAYA cancer improve quality life reduce burden late effects on survivors, health care systems society. The European Union-funded PanCareFollowUp project, initiated by Pan-European Network Care Survivors after Childhood Adolescent Cancer, was established facilitate person-centred survivorship across Europe.The Intervention co-developed with as part project. It a approach supported guidelines flexibility adapt local settings. consists three steps: (1) previsit completion Survivor Questionnaire (by survivor) Treatment Summary provider [HCP]), (2) clinic visit including shared decision-making, (3) call finalise individualised Survivorship Plan.We developed key components Intervention: Questionnaire, template, Plan educational materials HCPs survivors.
