Risk factors for placenta accreta spectrum in pregnancies conceived after frozen–thawed embryo transfer in a hormone replacement cycle

ABSTRACT
Objective Assisted reproductive technology (ART), especially frozen–thawed embryo transfer (FET) in a hormone replacement cycle (HRC), is risk factor for placenta accreta spectrum (PAS). This study aimed to clarify the factors PAS related maternal background and ART techniques pregnancies achieved after FET an HRC. Study Design We performed case–control two tertiary perinatal centres Japan. Among 14,028 patients who delivered at ≥24 weeks of gestation or were transferred delivery between 2010 2021, 972 conceived with 13,056 without ART. was diagnosed on basis FIGO classification clinical diagnosis retained products conception gestation. excluded women fresh transfer, spontaneous ovulatory cycle, donor oocyte missing details treatment. Finally, among HRC, 62 340 included this study. Multivariate logistic regression models used comparisons, adjustment age delivery, parity, endometriosis adenomyosis, number previous uterine surgeries caesarean section, myomectomy, endometrial polypectomy curettage, previa, stage embryos, thickness initiation progestin administration. Results associated ≥2 (adjusted odds ratio, 3.57; 95% confidence interval, 1.60–7.97) (blastocysts: adjusted 2.89; 1.15–7.26). In <2 surgeries, <7.0 mm 5.18; 1.10–24.44). Conclusion Multiple blastocysts are thin endometrium before also these pregnancies.
