ABSTRACT
Recurrent herniation is a well-recognised complication following the repair of congenital diaphragmatic hernia (CDH). There no consensus on optimal surgical approach to manage recurrent CDH. We present case laparoscopic correction using non-absorbable patch. A 13-month old girl presented outpatients department with constipation and breath-holding attacks, previously having undergone left CDH surgery 7th day life. The original operation was performed via subcostal incision primary carried out interrupted sutures. Chest X-ray demonstrated elevation hemi diaphragm posterior 'bulge' suggesting recurrence. An elective diagnostic laparoscopy confirming recurrence transverse colon through postero-lateral defect size 5 × cm complete absence noted rim. were significant adhesions anterior abdominal wall, including spleen, which divided mobilised haemostatically allow assessment defect. patch synthetic mesh anchored Total operating time under 120 minutes. patient discharged 3rd post-operative minimal analgesic requirements evidence follow-up radiographs. Laparoscopic safe effective option for cases technically feasible despite operative challenges that surgeon will inevitably face as in this case.
