Malignant struma ovarii is an exceedingly rare pathology with a paucity of established criteria regarding management and surveillance recommendations largely based on case reports retrospective data. Many authors have supported stratification malignant into low vs high-risk disease more conservative reserved for those deemed low-risk. Here we present unique recurrent metastatic after was undertaken in the setting initially low-risk disease.
