Application of next-generation metagenomic sequencing in the diagnosis and treatment of acute spinal infections

ABSTRACT
The purpose of this study was to verify the value metagenomic next-generation sequencing (mNGS) in detecting pathogens causing acute spinal infection by reviewing results mNGS 114 patients.A total patients were included from our hospital. Samples (tissue/blood) sent for detection, and remaining samples microbiology laboratory pathogen culture, smear, histopathological analysis, other tests. Patients' medical records reviewed determine their rates time needed, guidance antibiotic treatment clinical outcomes.mNGS showed a satisfying diagnostic positive percent agreement 84.91% (95% confidence interval (CI): 6.34%-96.7%), compared 30.19% CI: 21.85%-39.99%) culture 43.40% 31.39%-49.97%) conventional methods (p < 0.0125), found 46 smear negative samples. required identification using ranged 29 h 53 h, which an advantage over (90.88 ± 8.33 h; P 0.05). also played important role optimizing regimens with obtained methods. success rate (TSR) mNGS-guided (20/24, 83.33%) significantly higher than that empirical antibiotics (13/23, 56.52%) (P 0.0001).mNGS shows promising potential pathogenic diagnosis infections may enable clinicians make more timely effective adjustments regimens.
