Optimizing the prognostic capacity of baseline 18F-FDG PET/CT metabolic parameters in extranodal natural killer/T-cell lymphoma by using relative and absolute thresholds

ABSTRACT
ObjectivesTo investigate the prognostic capacity of baseline 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) metabolic parameters in extranodal natural killer/T-cell lymphoma (ENKTCL), and influence relative thresholds (RT) absolute (AT) selection on capacity.Materials methodsMetabolic tumor volume (MTV)-based were defined using RTs (41 % or 25 maximum standardized uptake value [SUVmax]), ATs (SUV 2.5, 3.0, 4.0, mean liver uptake) 133 patients. Metabolic classified into avidity-related (SUVmax, SUV [SUVmean], standard deviation [SUVsd]), volume-related (RT-MTV), avidity- (total lesion glycolysis [TLG] AT-MTV). The effects different threshold types (RT vs. AT) evaluated.ResultsAll moderately associated with prognosis. However, area under receiver operating characteristic curve MTV TLG was slightly higher than that for predicting 5-year progression-free survival (PFS) (0.614–0.705 0.563–0.609) overall (OS) (0.670–0.748 0.562–0.593). Correlations differed between (r < 0.06, P = 0.324–0.985) 0.56–0.84, ≤ 0.001). AT-MTV optimal predictor PFS OS, while RT-TLG PFS, combination RT-MTV SUVmax OS.ConclusionThe incorporation avidity significantly improved PET ENKTCL. Composite encompassed both recommended.
