ABSTRACT
Preeclampsia still represents a life-threatening pregnancy complication, associated with severe maternal and neonatal morbidity mortality. Low-dose Aspirin is advised to avoid preeclampsia in high-risk pregnancies worldwide. As does not cover all women at risk, the prescription raises questions concerning optimal target population, dosage, onset of therapy. The aim this study was test platelet responsiveness on by optical aggegrometry, gain robust biochemically assessment data an obstetric cohort. 248 high risk for development were included study. Aspirin-prophylaxis administered either 100 mg (n = 229) or 150 90) daily. Dosing maintained if testing revealed sufficient inhibition. If inhibition insufficient, dosage increased re-testing advised. 91 patients (91/229 39.7%) presented inhibitory effect mg, but 138 LTA showed inadequate response (138/229 60.3%). In 19 as starting dose due new recommendations. Of 64 did properly respond (35.4%). overall rate responding regardless 64.6%. This demonstrates insufficient aggregation about 1/3 even daily, who might potentially benefit from further increase. These show, that there need research allow personalized approach individualized therapy, maximizing preventive mother child.
