Cause Analysis and Countermeasures of Locomotive Runaway Accident based on Fault Tree Analysis Method

August 12, 2011, a locomotive runaway has happened on the local railway named Xihe of certain city, it led to death an older person and serious damage four automobiles. The causing factors accident have been comprehensively analyzed, which respectively include aspects: human, machine, environment management. By using principle fault tree analysis (FTA), set up after investigation. Based corresponding formula, authors calculated structure importance coefficient each basic event. analyzing causation events, minimal path sets obtained, order also obtained. Through above analysis, reasons this analyzed systematically safety countermeasures proposed. thesis can provide reference for prevention similar accidents.
