ABSTRACT
<b>Background and study aims:</b> There have been significant advances in the management of complex colorectal polyps. Previous failed resection or polyp recurrence is associated with fibrosis, making endoscopic extremely challenging; traditional approach to these lesions surgery. The aim this was evaluate efficacy a novel, knife-assisted snare (KAR) technique scarred colonic <b>Patients methods:</b> This prospective cohort patients, whom KAR used resect polyps > 2 cm size. Patients had previously undergone mucosal (EMR) developed recurrence, EMR attempted but aborted as result technical difficulty. <b>Results:</b> A total 42 patients underwent large (median 40 mm) Surgery for benign disease avoided 38 41 (93 %). No life-threatening complications occurred. Recurrence seen six (16 %), five further resection. overall cure rate 90 %. <b>Conclusions: </b>KAR by an expert endoscopist effective safe low rates.
