ABSTRACT
Background: Hospital care of older adults with frailty is complex.The post-take ward round (PTWR) a key moment at which this complexity can be managed.However, in the busy environment an acute take it difficult to consistently systematic and address all points suggested by joint college guidelines on round.Checklist methodology was applied attempt improve consistency documentation within Geriatric Assessment Unit.Innovation: A checklist containing 12 items deemed essential effective PTWR developed form sticker placed patient's clinical record.The acts as prompt allows any uncompleted task allocated specific team member for completion end PTWR.Evaluation: Pre-intervention data were collected direct observation over four week period PTWR.Consideration assessed prior introduction senior clinicians blind contents.The then introduced three consultant teams.Post-intervention period.Hypothesis testing proportions carried out using Chi-squared test.Data from six covering 41 patients pre-intervention seven 44 post-intervention.Comparison selected pre-and post-intervention are shown Table 1.
