ABSTRACT
PURPOSE: Near-infrared spectroscopy (NIRS) monitoring devices are a widely used adjunct to serial physical exams for microsurgical flap monitoring. Currently available two wavelength NIRS do not directly measure tissue ischemia but use the relative amount of oxidized hemoglobin deoxyhemoglobin calculate blood oxygen saturation (StO2). Although clinically improve early detection compromise, StO2 readings have significant artifactual variation requiring vigilant clinician interpretation avoid unnecessary re-exploration. We previously reported performance characteristics three device designed detect oxidation state cytochrome aa3, which is direct cellular ischemia. The lessened using an established and reperfusion model, did adequately reflect aa3 state. objective this study was compare broadband white light (> 1000 wavelengths) standard (ViOptix Inc, Freemont CA) commonly employed by measuring variability during corresponding – METHODS: devices, ViOptix device, were applied hands human volunteers (n = 20) pressure cuff placed around upper arm occlude arterial venous flow, as described. Measurements obtained from both at baseline, induced ischemia, 30-second intervals. measured between correlation broadband-reported changes. RESULTS: measurements proportionally decreased with similar variability. changing reduced began even baseline placement before decreased. Once reporting phases began, correlated devices. CONCLUSIONS: performed similarly regard ability surrogate marker However, also provided capability through Interestingly oxidation-reduction sensitive minor changes in perfusion. Future work will evaluate critical differences model.
