ABSTRACT
: Recent studies of rare cases pT3a renal cell carcinoma extending directly into the adrenal gland showed worse survival than in other and recategorization as stage pT4 was suggested. We assessed prognostic validity a diagnosis based on perirenal fat infiltration.: The records 1,794 patients with who underwent surgical resection between 1975 2000 at our institution were analyzed retrospectively. Focusing tumors, defined by infiltration, numerous clinical histopathological parameters investigated univariate multivariate statistical methods cancer specific primary end point.: identified 237 classified having disease. In tumors tumor size significant parameter predicting survival. most cutoff value for disease 7 cm. Patients distant metastasis had prognosis independent T classification. Therefore, to assess current classification regard T3a we excluded cM+ further subgroup analysis. Survival comparison pT1 pNall, cM0 (744 cases) cm or less (100 237) well pT2 (265 1,794) greater (93 yielded similar results. After splitting modified T1/T2 difference 5-year analysis found (pT1 plus 90% vs 73%, p <0.001). Subsequently all that an predictor survival.: suggest revising infiltration but rendering pT1/pT2 classification, which resolves without loss validity. Perirenal should not be used assign category. Tumors infiltrating reclassified T4.
