Surgical treatment with nephron-sparing procedures indicated only in symptomatic cases few been published literature. The aim this video illustrate step-by-step fashion diagnosis minimally invasive management entity paediatric patient. METHODS: We present 4-year-old boy 6-month history recurrent left flank pain isolated calix dilation on ultrasound. Nuclear Scan demonstrated tracer retention CT scan showed anterior posterior branches apical artery. Patient was positioned Galdakao-modified supine Valdivia position. retrograde pyelogram confirmed an obstructed long infundibulum. transperitoneal laparoscopic caliconeopyelostomy performed ventrally vessels. RESULTS: Operative time 90 minutes. discharge after 48 h. Double-J ureteral stent removed 1 month. There were no complications reduction ultrasound resolution symptoms during follow-up. CONCLUSIONS: obstruction should be suspected presence pain, haematuria, or urinary tract infection dilatation images. Laparoscopic surgery feasible option first line patients.
