Is There a Gap Between Program Directorsʼ Expectations and Residentsʼ Performance? Comparing Predictions With Outcomes for Competence in Patient Care

ABSTRACT
To determine whether a gap exists between program directors' expectations for performance and residents' actual on learning objectives designed to address the Accreditation Council Graduate Medical Education's (ACGME's) competencies.In 2007-2008, authors analyzed data of 40 pediatric residents across all levels training who were assessed total 8,974 times during 2002-2005 35 developed ACGME patient care competency. Residents "passed" if assessments met expected standards each level that set by directors in previous survey. concurrent validity, also reviewed faculty evaluations resident using modified version American Board Internal Medicine competency card.The mean pass rate (percentage meeting or exceeding predetermined standard) was 92% first-year, 84% second-year, 72% third-year residents. Actual except those procedural competence some related management. However, card demonstrated exceeded care, suggesting unrealistic these few items.Program predicted majority objectives. The offer lessons learned inform next iteration clinical competence.
