ABSTRACT
Retrospective radiographic review. To evaluate the outcome of selective thoracic fusion (STF) by using cantilever bending technique (CBT) and direct vertebral rotation (DVR) for major thoracic-compensatory lumbar (MTCL) curves selected new curve criteria, which are broader than Lenke criteria STF.Surgical treatment MTCL aims to maximize number that can be treated with STF optimize instrumented spontaneous correction. Comparing current guidelines shows surgical utilized may affect successful correction.Seventy-eight consecutive idiopathic scoliosis patients "C" modifier who met following three criteria: (1) main (MT) compensatory (CL) ratios Cobb magnitude apical translation (AVT) greater one; (2) MT/CL ratio flexibility less (3) 35° on side bending, were CBT DVR. Radiographs analyzed before surgery, immediately after at most recent follow-up (range, 2-5 years). All 78 successfully A mean 61% correction was matched 55% follow-up. Spontaneous AVT occurred in all patients. Global coronal imbalance common surgery (mean, 14 mm) remained so 12 mm). There 49 did not meet STF.
