Wang-Zwische Double Lumen Cannula—Toward a Percutaneous and Ambulatory Paracorporeal Artificial Lung

ABSTRACT
We are developing a high performance double lumen cannula (DLC) for minimally invasive, ambulatory and percutaneous paracorporeal artificial lung (PAL). The Wang-Zwische (W-Z) DLC was designed insertion into the Internal Jugular (IJ) vein with drainage open to both superior vena cava (SVC) inferior (IVC) maximizing venous drainage. A separate collapsible but nondistensible membrane infusion right atrium (RA) achieves minimal recirculation allowing total gas exchange. W-Z prototypes made by proprietary dip molding process "molded in" flat wire spiral stainless steel spring resulting in flexible yet kink resistant thin wall (0.1 mm) outer one piece construction. With ultra collapsed, an introducer shaft fits tightly within facilitate placement at SVC-RA-IVC junction. were tested while connected compact pump-gas exchanger circuit three sheep (2 acute 15 day study). Insertion simple, using standard techniques. Recirculation as low 2%. study demonstrated our prototype 26 Fr can achieve 2 L/min blood flow recirculation. design minimizes rate, maximizes cross-sectional area, achievable enhance exchange site venovenous support.
