ABSTRACT
Significance of tertiary Gleason pattern/grade 5 on prostatectomy has been studied, but its significance biopsy remains uncertain. Recent International Society Urological Pathology consensus conference recommended that score is generated by adding grade to the primary grade. We examined preoperative clinical and findings in 53 patients with pattern 119 primary/secondary 5. Prostatectomy prostate-specific antigen (PSA) failure rates were compared surgically treated patients. Cause-specific all-cause mortality nonsurgically. At presentation, age, gland volume, PSA, cancer volume similar Only 20 underwent 152 Regardless pattern, younger (P=0.003), had lower PSA (P=0.001), less (P=0.0001). failures not significantly different versus In nonsurgically patients, those a higher risk [adjusted hazard ratio (HR): 2.33, 95% confidence interval (CI): 1.10-4.90, P=0.026] cause-specific (adjusted HR: 7.52, CI: 2.84-19.87, P<0.001). contrast, secondary comparable 1.04, 0.47-2.32, P=0.92), marginally than 2.13, 0.75-6.10, P=0.16).
