ABSTRACT
The aim of this study was to investigate in a retrospective setting the patients' profile and results those undergoing surgery for hepatocellular carcinoma (HCC) high-volume surgical centers throughout world.Whether HCC is suitable approach which subset patients still controversial. EASL/AASLD (European Association Study Liver Disease/American Disease) guidelines, based on Barcelona Clinic Cancer (BCLC) classification, leave little room hepatic resection; inversely, other reports promote its wider application. On basis network "Hepatocellular Carcinoma: Eastern & Western Experiences," data 2046 consecutive resected 10 were collected. According BCLC 1012 (50%) 0-A, 737 (36%) B, 297 (14%) C. Analysis overall survival disease-free multivariate analysis prognostic factors performed. The 90-day mortality rate 2.7%. Overall morbidity 42%. After median follow-up 25 months (range, 1-209 months), 1-, 3-, 5-year rates 95%, 80%, 61% 0-A; 88%, 71%, 57% B; 76%, 49%, 38% C (P = 0.000). as follows: 77%, 41%, 21% 63%, 38%, 27% 46%, 28%, 18% identified bilirubin, cirrhosis, esophageal varices, tumor size, macrovascular invasion be statistical independent survival. This large multicentric survey shows that current practice widely applied among with multinodular, large, invasive HCC, providing acceptable short- long-term justifying an update therapeutic guidelines sense.
