Upon discharge, continued grow in size became more fluid than requiring repeat centesis draining. then re‐presented 3 weeks age surgical removal. In surgery, direct ligated. Histopathology demonstrated that lining composed squamous epithelium goblet cells occasional ciliated cells. location, morphological features congenital presentation were consistent paratracheal air (PAC). Surgical resection resulted excellent functional cosmetic outcome. Although not previously reported horses, PAC should be included differential diagnosis an equine neonates. Complete excision may result successful
