Abstract Background Persistent low‐grade fever has been observed in some patients during intravenous immunoglobulin (IVIG) therapy for Kawasaki disease (KD); however, smoldering (SF) not previously reported with KD. This study aimed to clarify the clinical characteristics of SF Methods A single‐center retrospective cohort study, which included a total 621 who received IVIG therapy, was conducted. Patients 37.5–38°C lasting ≥3 days after 2 initial‐IVIG were defined as group. divided into four groups according course: ( n = 14), biphasic (BF, 78), non‐fever (NF, 384), and persistent (PF, 145). The features described compared between groups. Results median duration group 16 days, longer than that any other neutrophil fraction higher BF NF but similar PF Repeated administration resulted increased IgG levels decreased serum albumin levels. In group, 29% had coronary artery lesions at 4 weeks. Conclusions frequency KD 2.3%. continued have moderate inflammatory responses. doses effective treating SF, acute occasionally observed. Active therapeutic intervention needed.
