ABSTRACT
<h3>Background</h3> Delayed discharge from hospital is a global problem with negative consequences for patients and the healthcare system. Common causes of delayed medically ready are issues such as lack appropriate social care or housing their needs. However, whilst there now plethora non-medical interventions which have been developed to facilitate timely remains evidence on impact. To bridge this gap in we aimed explore impact support coordinator (HSC) processes, service user outcomes costs. The aim HSC was work related improve reduce chances readmission. <h3>Methods</h3> We conducted mixed methods process evaluation two UK trusts employing semi structured interviews users, staff (=16), secondary analysis routine patient data (N=488) an economic assess Framework undertaken qualitative using initial framework discussions NHS partners.
