Irregular pulsation of intracranial unruptured aneurysm detected by four-dimensional CT angiography is associated with increased estimated rupture risk and conventional risk factors

ABSTRACT
Background Intracranial aneurysms (IAs) are common in the population and current imaging-based rupture risk assessment needs to be refined. We aimed use four-dimensional CT angiography (4D-CTA) investigate associations of irregular pulsation IAs with conventional factors estimated risk. Methods One hundred five patients 117 asymptomatic underwent 4D-CTA. Geometric morphologic parameters were measured presence (defined as a temporary focal protuberance ≥1 mm on more than three successive frames) was identified 4D-CTA movies. One- 5 year aneurysm using UCAS PHASES calculators. Univariate multivariate analyses performed associated pulsation. Results Irregular observed 41.0% (48/117) IAs. Aneurysm size (OR=1.380, 95% CI 1.165 1.634), shape (OR=3.737, 1.108 12.608), internal carotid artery location (OR=0.151, 0.056 0.403) independently (P<0.05). Aneurysms had 6-fold higher (1- 5-year [95% CI], 1.56% [0.42%–3.91%], 2.40% [1.30%–4.30%], respectively) without (0.23% [0.14%–0.78%] 0.40% [0.40%–1.30%], (P<0.001). Conclusions larger size, irregular-shape, non-ICA origin, have 1- should validated future longitudinal studies determine its predictive value for growth rupture.
