Giant cell myocarditis is a rare and often fatal disease. The most obvious presentation described in the literature one of rapid hemodynamic deterioration due to cardiogenic shock necessitating urgent consideration mechanical circulatory support heart transplantation. We present case 60-year-old man whose initial was consistent with myopericarditis but who went on develop decline left ventricular systolic function without overt compromise or dramatic symptomatology. confirmed via endomyocardial biopsy. Combined immunosuppression corticosteroids calcineurin inhibitor resulted resolution symptoms sustained recovery year later. Our highlights that giant does not always should be considered evaluation new onset cardiomyopathy uncertain etiology as timely diagnosis has distinct clinical implications management prognosis.
