ABSTRACT
Leptospirosis is an exceptionally rare infectious disease in the Republic of Ireland. can present with or mimic thrombotic microangiopathies (TTP/HUS). A 48-year-old male presented to a peripheral hospital short history diarrhoea, anaemia, hyperbilirubinemia, raised lactate dehydrogenase, thrombocytopenia, and severe acute kidney injury was transferred our tertiary care centre. form microangiopathy (TMA) suspected. However, no schistocytes were seen on blood film, reticulocyte count depressed. biopsy performed before initiating any potential treatment which revealed interstitial nephritis (AIN). considered subsequently serologically confirmed. The patient managed antimicrobials supportive therapy. Acute common leptospirosis often due AIN. Initial presentation TMA; however, differential diagnosis should be even nonendemic areas re-emergence disease.
