ABSTRACT
Background: Up to 15% of patients with partial Atrioventricular Septal Defects ( p AVSD) require re-repair the Left AtrioVentricular Valve (LAVV), 30% having significant regurgitation. Previous qualitative studies suggest abnormalities in papillary muscles, shortened chordae and thick chords as features associated LAVVR. We aim quantitatively assess sub valvar apparatus pAVSD using three-Dimensional Echocardiography (3DE) for its association Hypothesis: hypothesize that larger muscle chordal thickness AVSD is post-operative Methods: This a prospective study between 2006 2018, aged 0-18 years. All underwent 3DE under sedation X7-2 probe on Philips iE33 ultrasound system. Utilizing offline QLAB multi-planar reconstruction (v.10.8.5) we measured anterolateral (APM) posteromedial (PPM) length; cross sectional area circumference; length mid systole. vena contracta LAVV diameter Apical 4 chamber 2D color Doppler. Cohort was divided into LAVVR vs. controls based contracta/LAVV annulus ratio cut off >0.25 (greatest one-third cohort), (n=22) Controls (n=44). variables are indexed Body Surface Area, expressed median [interquartile range] or mean (standard deviation). Results: group were younger at initial surgery, had muscles thicker chords. Table 1 summarizes Control Eighteen re-repair, 14 (63%) (9%) Controls. Median follow up 1.5 time 1.3 Conclusion: greater risk subsequently first repair length, circumference
