Tachycardia-Induced J-Wave Changes in Patients With and Without Idiopathic Ventricular Fibrillation

ABSTRACT
Background— To know the underlying mechanisms of J waves, response to atrial pacing was studied in patients with idiopathic ventricular fibrillation (IVF) and non-IVF. Methods Results— In 8 IVF, J-wave amplitude measured before, during, after pacing. All had episodes without structural heart disease. The responses waves were compared those 17 non-IVF control subjects who revealed but no history cardiac arrest underwent electrophysiological study. IVF younger than (28±10 versus 52±14 years, respectively; P =0.002) larger more extensive distribution. decreased from 0.35±0.26 0.22±0.23 mV ( =0.025) when RR intervals shortened 782±88 573±162 ms =0.001). A decrease (≥0.05 mV) observed 6 patients. addition, 1 patient showed a distinct reduction unipolar epicardial leads. contrast, augmented 0.27±0.09 0.38±0.10 <0.001): 9 unchanged subjects. different patterns rapid suggest mechanisms: early repolarization conduction delay Conclusions— between patients, which suggests presence for genesis waves.
