ABSTRACT
Background— Serum creatinine and related estimating equations predict cardiovascular events mortality among persons with coronary heart disease (CHD). Cystatin C is a novel sensitive endogenous marker of kidney function. Whether cystatin concentrations are associated adverse ambulatory CHD unknown. Methods Results— Nine hundred ninety were categorized into quartiles serum at inception, ≤0.91 mg/L constituting the lowest quartile (I) ≥1.30 highest (IV). Cox proportional hazards models evaluated time to all-cause mortality, (composite death, myocardial infarction, stroke), incident failure. After median follow-up 37 months, 132 participants (13%) died, 101 (10%) had events, 57 (7%) Compared in quartile, those increased risk (hazard ratio, 3.6; 95% CI, 1.8 7.0), 2.0; 1.0 3.8), failure 2.6; 6.9) analyses adjusted for traditional factors. predicted similar these outcomes lower (≤60 mL/min per 1.73 m 2 ) or higher estimated glomerular filtration rate without microalbuminuria. Conclusions— High substantial risks CHD. This not completely captured by measures function routinely used clinical practice.
