ABSTRACT
We hypothesized that annuloplasty ring implantation alters mitral annular strains in a normal beating ovine heart preparation.Sheep had 16 radiopaque markers sewn equally spaced around the annulus. Edwards Cosgrove partial flexible band (COS; n=12), St Jude complete rigid saddle-shaped (RSA; n=10), Carpentier-Edwards Physio (PHY; n=11), IMR ETlogix (ETL; and GeoForm (GEO; n=12) rings were implanted releasable fashion. Four-dimensional marker coordinates obtained using biplane videofluoroscopy with inserted (ring) after release (control). From coordinates, functional spatio-temporal representation of each annulus was generated through best fit piecewise cubic Hermitian splines. Absolute total calculated from relative change length tangent vector to curve as occurring control state at end-systole. In addition, average Green-Lagrange end-systole along calculated. smallest for COS greatest ETL. Strains RSA, PHY, GEO similar. Except septal segment, all induced compressive entire annulus, values lateral segment. In healthy, hearts, (COS, ETL, GEO) induce are predominate region, bands (COS) an asymmetrical type intrinsic septal-lateral downsizing (ETL). However, most drastic (GEO) introduced similar physiologically shaped (RSA PHY), indicating effects on strain profiles cannot be estimated degree downsizing.
