ABSTRACT
Introduction: There is lack of knowledge on whether symptomatic steno-occlusion (SYSO), common in acute ischemic stroke (AIS) patients with atrial fibrillation (AF), could increase the long-term risk recurrence these patients. Methods: From a prospective cohort AIS and AF enrolled 14 centers between Oct 2017 Dec 2018, we identified who underwent MR angiography during hospitalization completed 3-year follow-up including death follow-up. SYSO was defined as (1) ≥ 50% stenosis or occlusion cerebral arteries relevant to infarction (2) any residual after endovascular treatment. Using cause-specific hazard models non-stroke competing risk, recurrent estimated according SYSO, respectively. Results: A total 889 (mean age, 74.4 years; men, 54.6 %; median NIHSS, 6) were analyzed for this study. During 1096 days follow-up, 152 strokes, 142 208 deaths observed. Patients compared those without, more likely be older, female, have hypertension, diabetes history stroke/TIA, antiplatelets at discharge less anticoagulants ( p <.05). The cumulative incidence without 25.2% 8.3% 1 month, 33.1% 9.9% year, 41.8% 13.1% 3 years, respectively <.001). With adjusting sex, diabetes, antiplatelets, anticoagulants, increased (adjusted ratio [95% confidence interval]; 3.02 [2.18-4.20]; <.001) (3.20 [2.28-4.51]; Conclusions: substantially by 3-fold more. Accordingly, should considered stratifying AF.
