ABSTRACT
Background: Black individuals are at a higher risk of stroke, but little is known about racial disparities in functional outcome. Using representative population-based study, we examined outcome over multiple timepoints the first year after acute ischemic stroke (AIS). Methods: We ascertained all hospitalized AIS events Greater Cincinnati Northern Kentucky region from July 2019-December 2020 (Black individuals) and January 2020-December (White individuals). Potential cases were identified by ICD codes, abstracted, physician adjudicated. Only patients who survived their initial hospitalization included. validated approach, modified Rankin Scale (mRS) was determined retrospectively discharge, 3 months, 6 1-year post-stroke. Mixed effect ordinal regression used to investigate race on outcome, adjustment for relevant factors. collapsed mRS scores 5 into one category shift analyses due small number with an 5. A race:time interaction term considered assess whether race-based differences varied timepoint. Missing data handled via full information maximum likelihood. Results: Over study period, 2541 survivors, whom 816 (32%) 1306 (51%) women (Graphic 1). Unadjusted distributions 1 follow-up shown Graphic 2. At 12 25.4% White (434/1707) 21.2% (169/799) had 0 or 1. When adjusted age sex, associated towards worse 3, cOR 1.48, 95% CI 1.30-1.68). other potential confounders added model, attenuated remained significant (cOR 1.16, 1.02-1.31). There no between time either model (0.10 0.09, respectively). Conclusions: studies long-term post-stroke outcomes U.S., found that experience outcomes, this incompletely when comorbidities management. Further research needed understand these driven modifiable social determinants impact rehabilitation.
