Rheumatoid arthritis associated autoantibodies in patients with synovitis of recent onset

ABSTRACT
STATEMENT OF FINDINGS: An inception cohort of 238 patients having peripheral joint synovitis less than 12 months duration was evaluated clinically and followed prospectively for 1 year to determine the clinical significance a number rheumatoid arthritis (RA) associated autoantibodies. Serum samples collected at time initial evaluation were tested factor (RF) antibodies Sa (anti-Sa), RA-33, (pro)filaggrin [antifilaggrin antibody (AFA)], cyclic citrullinated peptide (anti-CCP), calpastatin, keratin [antikeratin (AKA)]. RF had sensitivity 66% specificity 87% RA. Anti-Sa, AFA, anti-CCP all more 90%, but 50% this diagnosis. Overall, there high degree correlation between AKA, anti-Sa or anti-CCP, being highest (odds ratio, 13.3; P < 0.001). Of 101 who positive least one these four autoantibodies, 57% only one. Finally, anti-SA identified subset predominantly male RA with severe, erosive disease. Anti-SA, AFA are specific early but, overall, have little additional diagnostic value over alone. Although may preferentially recognize antigens, modest concordance them in individual patient sera suggests that it is unlikely single antigen involved generating responses.
