ABSTRACT
We aimed to evaluate the clinical and radiological short-medium term outcomes for treatment of bilateral osteonecrosis femoral head (ONFH) with hip-preserving surgery core decompression followed by tightly impaction bone grafting combining non-vascularized fibular allografting in one hip concurrent one-stage total arthroplasty (THA) contralateral side. hypothesized aforementioned showed benefits protecting preserved from collapsing thereafter THA was delayed or avoided. We retrospectively reviewed a consecutive series 18 non-traumatic ONFH patients (36 hips) who had undergone previous mentioned surgeries between July 2004 June 2013. Preoperative last follow-up Harris Hip Score (HHS) Visual Analogue Scale (VAS) were obtained evaluation X-rays antero-posterior frog-leg lateral views hips compared assessment. All telephone contacted out-patient clinic return visit at an average time 53.3 months (ranged 20 107 months). Of (15 men 3 women), there 5 diagnosed preoperative IIB stages according classification Association Research Circulation Osseuse (ARCO) remaining 13 ARCO IIIC stages. The mean age included 40.7 years (range 22 59 years). No followed-up difference existed genders. postoperative HHS 83.8 ± 17.9 points, it revealed statistical significance when 61.6 17.0 points (p < 0.05). VAS scores reduced 6.2 2.0 2.8 2.3 which also manifested From aspects, 14 acquired well repairmen necrotic areas head. However, other 4 ultimately suffered collapse, severe pain gotten rid after performed. The un-collapsed can achieve biological stability sufficient blood supply through obtain longtime as early non-weight-bearing function training, distributing whole body weight load THA. Consequently, we recommend this sort practice trial faced different though longer larger samples are essentially needed address its efficacy.
