ABSTRACT
The presence of a fourth aortic valve cusp (quadricupsid valve) is rare congenital malformation and often accompanied by other anomalies the adjacent cardiovascular structures. Among these concomitant anomalies, simultaneous association both single coronary ostium aneurysmal ascending dilation in combination with quadricupsid has not been reported yet. We experienced case 56-year-old female patient presenting as regurgitation resulted from malcoaptation valve. had also accompanying dilatation ostial anomaly. Surgical correction (aortic replacement mechanical devices supracoronary prosthetic graft) was performed without any complications. technological development preoperative imaging studies enable physician to encounter quadricuspid associated malformations more unlike previous reports. With review on valve, we discussed surgical considerations for treatment this
