Conversion surgery for undifferentiated carcinoma with osteoclast-like giant cells of the pancreas: a case report

ABSTRACT
Undifferentiated carcinoma with osteoclast-like giant cells (UCOGCs) is a rare subtype of pancreatic cancer (PC), and its clinicopathological characteristics are still unclear. Herein, we report case initially unresectable UCOGC that was successfully resected after FOLFIRINOX therapy.A 63-year-old man referred to us for evaluation mass detected by computed tomography (CT) during medical checkup. Computed showed 7.5-cm tumor located in the head body, which involved common hepatic artery (CHA), gastroduodenal (GDA), main portal vein (PV) thrombus. suspected endoscopic ultrasonography-guided fine needle aspiration, patient diagnosed locally advanced cancer. After ten cycles FOLFIRINOX, size decreased 3 cm thrombus trunk had disappeared follow-up CT scan. However, experienced severe adverse drug reactions, including neutropenia liver dysfunction. Therefore, performed pancreatoduodenectomy resection. The pathological diagnosis negative margin. He treated remains recurrence-free 6 months surgery.We undergoing conversion surgery UCOGC, resulted R0 could be possible regimen achieve UCOGC.
