ABSTRACT
Repeat surgery has usually been considered the first choice to solve paravalvular leaks of prosthetic valves, but it carries a high operative risk, mortality rate and an increased risk for re-leaks. Percutaneous closure such defects is possible, different approaches devices are used this purpose. For mitral leaks, constructing arterio-venous wire loop delivering device through antegrade approach most commonly technique. Transcatheter can also be performed transapical or retrograde transfemoral arterial approach. We present case 68-year-old man with leak that was successfully closed using Amplatzer(®) Duct Occluder II, via under three-dimensional transesophageal echocardiographic guidance, without use loop. The initial attempt cross defect unsuccessful, obstacle finally overcome by introducing complex interventional techniques.
