ABSTRACT
A retrospective study was performed to define clinical characteristics and therapeutic outcome for patients with large-cell immunoblastic lymphoma of the mediastinum.Fifty-seven who presented primary, mediastinal were retrospectively studied determine initial sites disease, radiologic characteristics, treatment, outcome, factors that have prognostic significance progression-free overall survival.Fifty-six 57 had disease confined above diaphragm. Bulky extensive intrathoracic infiltration common in these patients. All treated intensive chemotherapy regimens, 44% received chest irradiation. The 5-year survival by Kaplan-Meier estimation 50% a freedom-from-relapse rate 45%. Predictors relapse after included presence pleural effusion (P = .015), number involved extranodal < .01), lactic dehydrogenase (LDH) ratio > 3.0 (LDH value/upper limit assay; P .04) as well an incomplete treatment response evidenced residual mass on radiograph .02) or persistent gallium 67 avidity .01) chemotherapy. decreased .001), .022), positive posttreatment 67Ga scan .027).Patients primary approximate chance surviving disease-free therapy. at presentation associated extremely poor outcome. Bulk per se negative predictive factor only without effusions when compared did not bulk disease. In addition, all involvement two more relapsed standard
