An outbreak of Group B Streptococcal CNS infection in Singapore: unusual clinical and MRI findings (P1.330)

ABSTRACT
OBJECTIVE To describe the clinical and initial neuroradiological manifestations of patients in an outbreak group B streptococcal (GBS) meningitis. BACKGROUND Since Dec 2014, many healthy non-pregnant adult with GBS infection were identified Singapore; was thought to be related consuming infected raw fish. DESIGN/METHODS The database Singapore Neurologic Infections Program (SNIP), a national multicentre study for surveillance infectious neurologic disease, reviewed select central nervous system (CNS) during outbreak. Cases diagnosed on basis features, cerebrospinal fluid (CSF) findings identification or isolation Streptococcus agalactiae blood CSF. Demographic, information obtained prospectively abstracted. RESULTS Fourteen (6 male, 8 female) identified; median age at disease onset 58 years (range 22 81). Common presenting symptoms included fever (100[percnt]), meningism (57[percnt]), headache (50[percnt]), encephalopathy (43[percnt]), focal neurological deficits (36[percnt]) seizures (14[percnt]). All community-dwelling previously except 2 diabetes mellitus (one patient had previous breast carcinoma), 1 each childhood epilepsy meningoencephalitis. Diffusion-weighted MRI abnormal 13 patients, showing tiny hyperintensities subarachnoid space ventricles 9 as well larger lesions within brain parenchyma patients. Five cerebellar abnormalities. CONCLUSIONS/RELEVANCE Among adults meningitis, diffusion-weighted helpful detecting small subarachnoid, ventricular lesions, including involvement.
