ABSTRACT
In Brief BACKGROUND: The treatment of postherpetic neuralgia (PHN) continues to be a challenge in clinical pain management. this randomized, controlled study, we assessed the effectiveness repetitive paravertebral injections with local anesthetics and steroids for prevention PHN patients acute herpes zoster. METHODS: One hundred thirty-two zoster diagnosed 1–7 days after onset rash were randomly assigned receive either standard therapy (oral antivirals analgesics) or additional mixture 10 mL 0.25% bupivacaine 40 mg methylprednisolone acetate every 48 h week. Efficacy was evaluated at 1, 3, 6, 12 mo end treatments. primary point proportion zoster-associated and/or allodynia 1 inclusion. Statistical analysis performed based on intent-to-treat population. RESULTS: thirteen completed 1-yr follow-up. At posttherapy, 13% group reported zoster-related pain, compared 45% (P < 0.001). incidence still significantly lower than group. quality life improved both groups each follow-up time no significant difference between groups. CONCLUSION: Repetitive anesthetic block combination plus acyclovir analgesics reduced alone. Published ahead print August 27, 2009
