ABSTRACT
Hashimoto's encephalopathy (HE) and anti N-methyl-D-aspartate receptor (NMDAR) encephalitis have clinical overlaps. A 70-year-old woman presented with acutely developed confusion, disorientations psychosis. HE was suspected based on goiter, markedly elevated anti-thyroglobulin anti-thyroid peroxidase antibody. She placed high dose steroid intravenous immunoglobulins administration, which did not ameliorate her symptoms. After the antibodies to NMDAR were identified, weekly 500 mg of rituximab 4 cycles started. The current followed up indicated a complete recovery. possible associations between antibody autoimmune thyroid in anti-NMDAR positive autoantibodies remain unclear. However, trend toward higher incidence patients than without has been observed. Cases only (pure encephalitis) also occur. Therefore, it is important for clinicians know pathogenic differences autoantibody pure relevant treatment, predicting prognosis, future follow-up.
