Significance of subepithelial deposits in patients diagnosed with IgA nephropathy

ABSTRACT
Subepithelial deposits are observed in rare adult IgA nephropathy (IgAN) cases and a key diagnostic finding IgA-dominant infection-related glomerulonephritis (IgA-IRGN). Sometimes, it is difficult to distinguish IgA-IRGN from IgAN without precise clinical history. We hypothesized that some might be diagnosed as with subepithelial (IgAN-SD) aimed clarify the significance of patients IgAN. examined 464 at Nagasaki University Hospital affiliated hospitals between 1996 2013. The differences clinicopathological findings IgAN-SD no (IgAN-NSD) were investigated. In addition data typical pathological features, we analyzed complement levels, immunoglobulin localization, light chain staining patterns, intramembranous deposits. There 214 men 250 women mean age 38.8 ± 18.3 years. deposition was 51 (11%). Compared IgAN-NSD, those had significantly lower serum protein (6.4 g/dL vs. 6.7 g/dL; p = 0.02), albumin (3.7 3.9 (C3) (94 mg/dL 103 mg/dL; 0.02) levels. Diffuse mesangial hypercellularity (M) (65% 45%; p<0.01), endocapillary hypercellularity: (E) (43% 28%; 0.03), glomerular capillary wall (22% 8%; p<0.01) more common IgAN-SD. incidence lambda predominance (47% 63%; 0.03).
