Efficacy and safety of guide extension catheter in balloon pulmonary angioplasty for treatment of complex lesions in chronic thromboembolic pulmonary hypertension

ABSTRACT
Background Balloon pulmonary angioplasty (BPA) is used for treatment of inoperable chronic thromboembolic hypertension (CTEPH) and residual after endarterectomy (PEA) to improve hemodynamics, right ventricular function, exercise capacity. However, the effectiveness safety guide extension catheters BPA in patients with CTEPH have not been demonstrated. Methods We retrospectively analyzed 91 lesions 55 sessions 28 who underwent using a catheter. The purpose (backup, coaxial, extension), efficacy, were explored. efficacy catheter was assessed based on success procedures evaluated procedure-related complications. Results Regarding intended use, strengthen backup force guiding 52% cases, extend tip 38% maintain coaxiality 10% cases. Procedural achieved 92.7% 95.6% lesions. Complex had lower rate than simple (p = 0.04). concerns, complications observed 5 (9.1%) 6 (6.6%). Only one case artery dissection reported. Except this case, catheter-related observed. Conclusions A can be safely anatomically complex branches by increasing support.
