ABSTRACT
The role of surgical treatment for malignant pleural mesothelioma (MPM) continues to be controversial. We carried out a retrospective review the prognosis in patients who had radical surgery MPM. Of 87 consecutive on whom exploration biopsy-proven MPM was performed, 31 underwent extrapleural pneumonectomy (EPP) and 34 pleurectomy/decortication (P/D). Sixty-five having EPP or P/D included 58 men (89%). median age 60 years (range 35-78) histologic type epithelial 48 (74%). IMIG staging classification p-stage I disease eight (12%), II 13 (20%), III 40 (62%) IV 4 (6%). Operative mortality 3.2% none P/D. 3-year survivals after were months 33% whereas those 17 24%, respectively. A multivariate analysis demonstrated that older (P=0.0467), non-epithelial histology (P=0.0057) III-IV (P=0.0019), but not gender, side, procedure, significant independent negative prognostic factors. Although appears acceptable early stages, we encourage EPP, en bloc resection without entering cavity with intent curability, which provides oncologically complete all disease.
